The Mediterranean Diet in 2021: Why the Evidence Keeps Growing

Of all the dietary patterns studied in rigorous clinical trials, the Mediterranean diet has accumulated perhaps the most compelling body of evidence for long-term health. The PREDIMED trial — a landmark Spanish study published in the New England Journal of Medicine involving nearly 7,500 participants at high cardiovascular risk — found that a Mediterranean diet supplemented with olive oil or nuts reduced major cardiovascular events by approximately 30 percent compared to a low-fat control diet. Subsequent research has linked the pattern to reduced risk of type 2 diabetes, cognitive decline and certain cancers.
What makes the Mediterranean diet so effective? It is likely the combination rather than any single component. Abundant vegetables, fruits, legumes and whole grains provide fiber, antioxidants and phytonutrients. Olive oil delivers anti-inflammatory monounsaturated fats. Fish, especially fatty fish, provides omega-3 fatty acids. Moderate consumption of nuts and seeds adds protein, healthy fats and magnesium. And the pattern’s inherent flexibility — it is a way of eating, not a rigid protocol — makes it sustainable in real life.
